I use Kydex knife sheaths 85-95% of the time. I don't often attach anything to the Kydex sheath.
If I feel like I should attach stuff to my knife sheath I strap a MOLLE utility pouch to the Kydex sheath. I have been using pouches by HSGI and ATS.
This is a Busse NMSFNO in Mashed Cat Kydex with an HSGI Mini Radio pouch attached to the paracord with Tactical Tailor MALICE clips. That's two pieces of green shock-cord to hold it down to the sheath really tight (not necessary in this instance) or to hold something else.
The same size pouch can be attached to a smaller knife in the same way or the small knife can be put on the front of the pouch. There are so many MOLLE pouches out there that can be used this way.

I have been farting around modding a Becker BK-16 casually now and then. And I have cut away a lot of the original sheath. The latest incarnation has these eyelets. I almost have the knife hanging vertically, which is my goal. I need to play around with the cord threading to get it right. I tried to neck hang my ESEE 4 and it felt like a boat anchor. The 16 feels fine to me. As soon as I heard about the 16 a few years ago, I wanted one as a necker. I still have a belt carry option but may slice all that off also. I like this knife.

Wow those are some nice set ups. Makes mine look pretty crappy by comparison but I'll show it any way. I bought a "primitive" knife at a rendevous the other day that didnt have a sheath but the maker gave me a piece of leather so I went to Hobby Lobby and bought a hand stitcher and this is what I came up with. It looks reaaalllly primitive.

This is what i do to make my stock sheaths 100% more usefull. I buy 4' lengths of 3/16" steel rod from HW Store & bend them up to make giant D-ring swivels. Then i use belt keeper/s to attatch to belt. Drop-leg swivel set ups are where it's at for me. VERY COMFORTABLE for all day wear while working. A leg strap helps it from slapping you all day.
Took the stock BK2 sheath and mounted it to an ESEE MOLLE back and made a drop leg rig. I later added a pouch to the front
How I used to carry my BK9 with a shoulder strap.
Custom kydex from AZWelke with the ESEE pouch on it(That's on my BK2). I'm working on a drop-leg for this too.

Well I didn't make this sheath for my Bush Cleaver, but I told him what I wanted Lamont Coombs made it happen tho.
Made this for a friends wood lore clone, also braided the leather lanyard
