Not sure what caused the above error. I was able to select 2 choices under 2. Product Selection. This appears to be in error. Choosing another choice should deselect the previous choice under Product Selection. All the other selections ( 1 and 3 through 11) all deselect correctly.
Otherwise looks ok to me.
