Custom Knife Questionere?

Hi everybody,
I want to put a questionere on my website to give me an initial overview of how a customer wants their custom knife made. So could you please give me some questions you might ask someone whos looking for a custom knife.

Example

Type of handle material?
Length of blade?
Type of metal?
Its main use?
How much are you willing to send?

What would you add?
 
IMO the personal touch is increasingly important.....I'd suggest
asking the customer the questions yourself, via phone or e.mail,
then YOU can make out the form if needed.
 
some peopkle are not bothered ringing though, i have a internet marketing buddy who makes millions every year and he said that would be the key for me getting new customers, they will fill out that info a lot quicker than ringing and then il have there name and number to chase up. He said he has tested this time over time and it makes all his websites more money.
 
i think both options (phone and email) are important. he more options u give the consumer, the more money u will make. let people decide for themsleves what they want. have them fill out a basic questionaire and then have them call u......?
