I've seen some custom knife handles described as 'engine turned'. What exactly does this mean?

I believe it is when the titanium whether used as the scale material or as a liner has those precision circles that seemed to be stamped into the metal overlapping each other. My Stan Fujisaka folder's gold andonized liners has that design on them.Hope this helps.

Its just like the Jeweling on rifle bolts. Some people use Cratex abrasive in the drill press, some use a round wire brush dipped in an abrasive/oil mix. You just push down with the drill press where you want the "fishscale" to be, and overlap the rest of the scales to the desired pattern. Engine Turning, Jewling, and Damasending are words for this.
