If you're lucky, UnknownVT may see your post and identify that SAK. If not you can probably find his email here somewhere.

The SAK appears to be a waiter, at least that is what a similar model is called now. Dont know what year, but as stated above, maybe UnknownVT will show up...he may know, or at least point you in the right direction on where to look.
