Accidental lock relaese

Hi guys. This may sound a bit stupid, but I am trying to figure out how most knife locks are accidentally unlocked and how the knife is postioned in the hand if and when this happens.I guess what I am getting at is that it seems to me the knife must be being used in a way other than typical cutting tasks, if it were to close on a hand resulting in a cut.I look at a couple of my knives from different companies with a couple different types of locks and cannot figure how my knife lock could fail when the knife is being used properly.Any thoughts on this? Thanks.Oh, I'm not talking about slip joints either, just to keep them out of the question.
 
Various grips put the fingers over the lock release so that in a tight grip it they can be disengaged. Icepick is a common problem for lockbacks and hammer for liner/integrals.

I think most locks fail from dirt. Some pocket lint in the lock area has been the biggest cause of problems for me. I've had the experience with every common lock type. I could tell because they didn't make the right noise when deployed.

As to failure of the mechanism, I would credit wear, assuming it was made right to start with.
