Any simple simple way of adding a penny to a rivet or something to attach t onto a leather sheath?

Solder...
I solder the penney to a Chicago Screw. Since you have to do the soldering before you attach the thing to the sheath ( to avoid burning the leather, nylon, kydex, etc), a penney soldered to a rivet may come loose when you 'set' the rivet. A CHicago screw ( binding post) solves that problem.

Do you think that epoxy would work? Perhaps if you countersunk the chicago screw a bit?

If your not married to the idea of using the penny on a piece of hardware I'd think about JB welding it onto a piece of flatstock like .025 then using it as an inlay with the flatstock sandwiched between two pieces of leather and a hole cut in the top leather to reveal the penny. You could also drill holes in the flatstock and stitch it in if that made you feel better.
Just a thought I figured I'd put out there

Yep, JB Weld is the best epoxy for this...as Josh said. Rough up the items with sandpaper, then degrease with lighter fluid, then epoxy. Also, be aware that pennys are no longer pure copper, but copper plated zinc. SO...vigorous sanding/roughing-up may remove the copper plate.
 
Get an older penny if you solder, I can't remember when they switched from solid copper to plated, but I think pre-87 is a safe bet.
 
Even the newer Cu plated zinc pennys will solder OK. But, over time, the Cu plating may wear off on the exposed side...although, the plating seems to be quite thick. I've never seen a post-'82 (zinc) penny with the Zn exposed due to wear.

It's an Aussie penny, though.
