Anyone lose anything to kydex?

I've noticed on the 6-7 kydex knife sheaths I own, they loosen up over use and time. Has anyone lost anything due to a kydex holder or sheath's poor retention?
 
No, I have not.

However, if I felt a kydex sheath was too loose, I would fix it. Heat it up with a hair dryer, or even brief exposure to an open flame from a lighter (I know – I'm terrible to recommend such a thing, but I've done it, and it works), and then squeeze it tightly while wearing an oven mitt while it cools.
 
Nope. I've not had kydex loosen up either except for some initial wear from sharp edges. If you have something with sharp corners I'd hit them lightly with sandpaper or those sharp corners will continue to cut the kydex indefinitely.

One thing I have noticed is a coated knife will loose some coating at contact points and that could lead to a looser fit also. If that's happening it would make sense to reheat and press tighter after the finish is mostly done wearing.
 
I've been using Kydex sheath for about an year and it holds well.
To be honest it was a bit loose from the beginning :D that I'll re-build it someday.
