An IWB loop can be added to one of the mounting holes in the sheath with the factory belt loop and hardware detached, but it's a pain to draw from as it's not smooth unless you use your thumb to release the knife.
You can modify the sheath with the factory loop for horizontal carry by taking the loop and hardware off, putting the loop around the sheath, then screwing it together with the metal strip, and original screws-the holes line up. I put the metal clip from an old Nimravus sheath on the loop, which makes the sheath versatile, but it's very bulky.
I could take some pics tomorrow if anyone is interested in seeing those setups. I really don't like the factory sheath much, though, and think a custom one is the way to go. I'm about to order some sheathmaking stuff so I can make my own kydex/concealex gear, and the 220 will be one of the first knives I try to make a better sheath for (once I get the initial trial and error phase over with
