There's also Chuddy Bear, Armorall leather, Mikey Moto, to name some all very good. + you have Condos listed above he posts pics a lot of his work here, looks really good.
x2 for Rowe and Chuddy Bear, they both make a really high quality sheath. I also want to throw Martin into the mix, http://www.martinsheaths.com/, they are by far my favorite if you are willing to spend the extra money to ship your knife back and forth to the Netherlands.

I'm more interested in a simple sheath, with belt loop.
The little brown one on the right, while simple, is the best sheath I've ever used. Snug, durable (dog chewed it), and easy to use.
You would really do yourself a favor by checking out Paul Long's sheaths. I'm not sure that he has an equal! Plus, he can have your sheath to you in under two weeks unless he is just truly swamped. Just search Paul Long sheaths on Google for some of his work. He doesn't have a website and he needs your knife to to do the build. However, if there is any way that you could take your knife to him it would be time well spent. I drove four hours to go see him then spent three hours in the shop with him. He is a true master craftsman. He is a member here so you might just search his name.
Chuttybear and Armorall are your best default sheath makers that are actually in business. Mikey Moto and Rainwalker seem to have the most innovative designs but I can't get either one of these guys to actually make a sheath. They don't even answer emails or fill previously placed orders. Chris Noe at Noe leather is excellent but I understand he is taking an indefinite break and Rowe Leather is also good as is Sunrise River. I can highly recommend Sunrise River but he does not have the variety of Chuttybear or Armorall. He's basically an excellent knife maker. Finally Savage leather is good but does not have a great deal of variety. Hope this helps. Buy Brown, Al Wilke and mashed cat are probably your best Kydex benders.
