Cord wrapped handle - how?

Any help how to do a cord wrapped handle. It looks great, and ensure perfect grip but ...
1) What cord should I use?
2) How to tie it?
3) It's just wrapped around a handle or glued to it also?
4) What is the initial and final knot?

Great page but what about traditional japanese wrapping with flat cord? I know how to tie a silk cord along a handle ("fold and cross" pattern found in BladeForums archive) but don't know the final knot that goes through hole in pommel.
Any help?
