Is the difference in grit very large between these two? I know the DMT coarse will cut faster than the grey Sharpmaker rods, but will it be hard to go from coarse to the medium ceramic? What if I were to use the DMT extra coarse?

The difference between DMT coarse and SharpMaker medium is not large. Extra-coarse to SM medium is a pretty large gap, so it will take some extra time on the ceramics to remove all the scratches the X-coarse hone leaves behind, if that is your concern.

Hm, well I definitely want the DMTs to remove metal a lot faster than the Spyerco medium, so I guess I'll go with the extra coarse. I can deal with doing some extra strokes on the ceramics, it doesn't take long to do say 100 a side.

Even the DMT coarse cuts a lot faster than the SM medium ceramic rods. I mostly use the extra-coarse for grinding out damage and setting new bevels.

Spyderco Medium abrasive equivalent is 20 microns.
Fine - 6 microns
UltraFine - 3 microns
DMT Fine - 25 microns
DMT Coarse - 45 microns.
DMT Extra Coarse - 60 microns
DMT Extra Extra Coarse - 120 microns

I usually goes
DMT XXC - 120 microns
DMT C - 45 microns
DMT EF - 9 microns
Green Rouge - 0.5 microns

and this works great for me.

To answer your question - I think you may start from XC or even XXC, because no big difference between 45 microns and 20 microns.

Thanks, Vassili.
