It usually means the knife has a scratch or other cosmetic blemish. Sometimes the knife may be more diffiicult to open or otherwise not as well finished. I know of no knife companies that would sell unsafe or structurally-flawed knives as "factory seconds". Additionally the knives are often, depending on the manufacturer, notched, stamped, or otherwise marked to distinguish their "Second" nature.

IMO, I would buy factory seconds as inexpensive users, never for a "keeper".

Very well defined, Kershaw uses small red x's over their name, and they are 99% of the time cosmetic and sometimes maybe even overstocks.
 
I just bought a Kershaw Seagal factory second and it is in better shape than my other Seagal! Thanks to you-know-who ;)
 
Some factory seconds are seconds because of a very minor error...something that may be off by thousandths or something....something that most people overlook or dont care about. Sometimes I've seen seconds that I had a very difficult time finding the flaw, and when i did, it was laughable. I'd certainly buy a second for regular use. usually they are significantly cheaper.

Cold Steel marks their seconds with a big number 2 stamped into the bottom of the handles usually. I'm not aware what other companies do. I imagine some companies are too proud to even allow a flawed knife to leave the shop.
 
Thank you all for the answers to my question. Now I know that it means knives with problems, but I'm still wondering how are knivemakers selling them. Do they let you know that it's a second before you buy it? Is it cheaper? I'm a guy who lives in Japan never seen anything like seconds at any stores in this country.
 
Ontario sells seconds at their factory store. They put a small brand (burn mark) on the handle. There are some very nice deals to be had for a user.
 
AG Russell also sells seconds of some models. They mark them with a notch on the tang, just behind the kick on a folder. Good values
 
Factory second also means that the warranty doesn't apply and sometimes you may not get the same sheath.
