Good knife blank supplier?

I am new to knife making and want to start with a blank made to a fairly standard outline. A few questions I have are:

Is there a good supplier for this type of part? (I think I can use either 440c or ATS-34 material 1/8" thick)

How much should this cost and how long should it take?

Will the blank already be heat treated?

If I want to develop my own shape can they do my design also?

Two custom makers that immediately come to mind are R.W. Clark and Mike Irie. These guys grind a precise blade and should be able to work with your designs. Their heat treat should be much better than what is available from the supply company's. On the other hand, I have had very good results with the blanks from Koval, K & G Supply and Sheffields Knife supply. Be careful though, they can be habit forming.
