Horizontal Sheath Advice

A friend of mine has seen some leather sheaths I have made for myself and asked me to make him a horizontal sheath for the small of the back for his vintage Puma Boot Knife. It will be made in leather.

I have never made a horizontal sheath and am concerned with retention as this knife holds sentiment with him. He is former LE so concealment and small of the back are not a concern. He was thinking wet formed about 1/2 to 2/3 the way up the scales. Is this enough coverage for a solid and continued retention without using a strap?

Also, he requested a way to adjust for differing belt widths. Possibly, hook and look sewn inside of a leather strap for a clean exterior appearance? Here are a few photos of said knife.

I would have to run a strap over the top of the handle for retention.
 
Here are some photos of finished sheaths of the type you mentioned. They address your concerns for both retention and multiple belt widths. These two examples happen to be for right hand draw, center of the back carry. Both feature a snap off belt loop which can be made to accommodate various belt widths. These are, of course fully leather lined so the metal studs and the retention strap are not exposed in the interior of the sheaths.

Paul, I appreciate the photos for visual reference. I have always lined my sheaths/holsters and was planning on lining this one also.
