Sharpen a karambit? My first thought would be to use a rod of some kind, other than that, I got no idea...
 
Ceramic rod, diamond rod, or any cylindrical object with abrasive on it will work. A section of pvc pipe, broom handle or a large(ish) dowel with wet/dry sandpaper wrapped around it would be almost ideal, and you can (if you wish) choose the size of the cylinder to closely approximate the radius of the recurve in the karambit's blade. I'd use edge-trailing strokes, as in a stropping motion, if using the sandpaper.

Flat hones of relatively narrow width will work too. You'll need to be careful with pressure, as only the edges/corners of a flat hone will be making contact, and will tend to focus & magnify pressure into a very small contact area on the blade's edge. That'll make edge rolling/chipping more likely, if pressure is too heavy.
