I don't understand the concept of a boot knife

Ironic considering that I actually wear* a boot knife, but I've never understood what I think of as the generally accepted use for a boot knife, namely as a hideout weapon for a bar fight. If you're wearing (presumably combat or cowboy) boots with jeans over them, how the heck do you get the thing out during a fight? And aren't they damn uncomfortable, rubbing against your leg for years until maybe that one time where you get into trouble and you have to actually use it?


* I keep a Candiru on the outside of my work boots. The thought is that since when I'm in the field it's usually the deep desert, either I need something for simple overnight survival tasks as a backup to my main knife and the truck breaks down, or someone gets the drop on me and I have the time to sneak it out of it's (velcro, uh...) sheath.
