Integral knife

I would like to make an integral knfe with the blade and handle all in one piece. Does someone know of a contact who could rough machine it for me? Sure would appreciate knowing.

Most any machine shop can do the basic milling to create the integral part from a bar of steel. You can do the blade shaping from there.
Besides a full-size diagram from the side and profile ... you will need a bar of steel as long as the final knife, as wide as the final blade, and as thick as the bevel thickness. This may be something like 10"X1.5"X3/4". Not every steel supplier has bars that thick.
 
I would look for a local machine shop
Those knives are usually forged because it allows for a more efficient use of material. As Stacy said if you mill it your starting material will need to be as wide as the final blade height and as thick as the bolster you are planning. Basically the knife needs to fit inside a rectangular prism of steel
Most (80%, 90%?) of that material will have to be removed by the mill so that will be expensive. I think work-holding is not a huge challenge here, but it's not trivial and that will add to the cost
An efficient design here would want a fairly narrow blade height wise. This approach would be more effective for a stiletto rather than a chef's knife
