The stud is used two ways. The simplest is just to shove the whole sheath between the pants and belt, or inside the waistband, and the stud keeps the sheath from going any deeper. It works well for casual or in-town carry. The best, is to punch a hole through the belt, so that you can slide the sheath underneath the belt, but outside the pants, and let the stud pass through the hole. Then the belt's tension holds the sheath in place, but it can be rotated to a comfortable angle. Bill Bagwell's Bowies, and many other Bowies from traditional makers, come with such sheaths in leather. This is the first time I have seen it in kydex. My Hell's Belle from Ontario came with this type of sheath. It is, of course, a custom collaboration with Bill Bagwell. Rowe makes this type of sheath in leather on a custom basis. I would have to do a search to find his website. He provides sheaths to several custom makers.
