This is my first axe project and I happen to come across a plumb 3/2 double bit that looks to be in good shape. After finally getting a piece of the old handle out. It was held in with about 15 nails, I found that the top of the axe is sort of mushroomed in where the handle will stick through. My question is should it be this way?

No. I should be flush. My guess is that whoever used the multiple nail wedging technique mushroomed it. When I've gotten one like that I usually grind it smooth with a dremel tool or round file.
 
Thanks for the help. I had actually started filing on it and then thought I should probably ask before I screw something up.
