I especially like the dainty hold on the knife as it comes out, kindof like how old ladies sip their tea.

I found that the best way to get it fully locked while pulling from the pocket is to bring it straight backwards, instead of the more upward motion u used.

Well, to be fair, I think the dude is holding like that so that the viewer can see the action; otherwise, part of the opening sequence would be obstructed by the hand...
