Texas Knifemakers Supply is where I've been getting mine. Their website also has the instructions posted and a copy is included with all kydex/concealex orders.
The instruction as best I can remember: heat kydex until soft, remove from heat quickly and place in press, place knife in between pieces of kydex or fold kydex over knife, and apply pressure until kydex is cool. After kydex is cool drill holes for rivets or chicago screws, and attach the pieces of together. Then take a heat gun or cigarette lighter and heat kydex in areas that need fine tuning and fine tune by hand(wearing some type of glove to protect hand or using tools). Sand sheath to finished shape.
