Look for a "sfilati" model by Italian maker Rocco Petrunti. VERY sweet looking single blade penknives.

Different French knife companies make a Laguiole knife in a pen knife version. 2 3/4" (70mm) closed, beautiful wood scales and jewel like construction. Do your homework as quality varies from company to company but they can be beautiful and very useful.
 
They're a bit out of your range, size wise, but Otter-Messer has some great looking options too, including this stainless Beemaster (blade length 85mm, handle length 105 mm):

Boker Tree Brand Barlow?
One of the remaining Sheffield makers mmight make a two blade in the size you want.
I think Victorinox/SAK makes a medium size 2 blade ... the "Flourist" or "Gardener"? Im not sure what they call it.
Would a 4 blade 2 layer SAK like the Cadet or Recruit ( Cadet has a fingernail file in place of the Recruit's punch/awl) work?
Neither have back side tools, and both do have the toothpick and tweezers stored in the covers.

I’ve never seen a Victorinox Recruit with an awl, just the secondary blade. If you have one I would be interested in seeing it.

I have several folders of various sizes made in Spain by Joker that I think are excellent knives. My smallest ones are 3.25 inches closed (about 8.25 cm); I don't know if Joker makes smaller models than that. Mine are all single-bladed, with horn or stag handles; a couple of mine are lockbacks.
