maddox question

For anyone who knows a lot about this knife, i notice its for stroke victims, or people with little wrist movement,. Knowing this, i dont understand how the maddox knife would be used, in a paint scraper motion?
 
As I understand it, it is meant to be used vertically with a slight rocking motion.
There is something similar that has two handles (one each side of a curved blade), that I believe originated in chinese cookery, but I can't remember it's name.
