Email Amy and Jerry at the provided emails.
Include all the info they need.
(real name, forum name, contact info, including phone, email etc, and shipping addy).
They will contact you eventually, when the knife is about ready for shipping, to bill you.
Will be about 10 weeks before any are ready to ship, longer if you wait till the last day to order. This will be open for order for only ten days total. So don't snooze.
Being a Hog is a simple process, but if you are not already one, you don't get the discount.
The process is described in several threads about becoming a hog. You can find them simply by searching for threads started by Jerry Busse, or Amy-0 (that is a zero).
