Need advice on my forge parts.

I would like your advice as too what parts I need to make my forge run?

Looks like a NC Tool Co shoer's forge. (Probably a Mamma)

You need a gas hose and propane regulator. They can be bought as a unit from High temp Tools and Refractory, www.hightemptools.com
Get a propane quick connect with a shut off valve from them ,too. This will allow fast and easy hook-up of the gas hose to the forge as well as on/off control. The shut-off valve part of the QC fitting goes on the threaded end of the manifold in your photo.

The little red button is the igniter, and may or may not still work, as they go bad often. You can get a replacement...or just use a long flex-neck lighter like most do.

Show us a photo of the inside and the ends, please. That will tell us what else you might need.
 
Stacy is correct, it does look like one of the NC Tool forges. It also looks like it might need to be relined. Go to the NC tool website at NCTOOLCO.COM and look at their forges and repair parts.
