New Username with Upgrade?

I have been a gold member in the past and let it lapse. I would like to re-sign up for a gold member again, but would like to know if I could change my username at the same time. I have a good reason for wanting to do so.

Sure, there's a procedure for getting your name changed, it's $10. We can do it at the same time as your gold membership

I followed an old link for name change, that charged $10. But it was a different place (and also $30) than the $25 gold membership if you click the link at the top of the page. So, I wasn't sure if I was in the right place. So, I decided to use the link at the top of the page, and there is no option for name change. What is the right way to do it?
