? on SARK origin?

I was reading in Tactical Knives about the history of the SARK knife. How it was created
to replace a knife that failed in a naval rescue attempt.

My question is, does anybody know what knife it was that failed and how, lock, blade, or handle?

my memory is bad since my accident, but i think it was either a spyderco or BM. I think the locks failed because it couldnt perform underwater. i havent taken my medication today, but in the morning i might remember more.
