I recently got a sheath made by Eric Noeldechen for my CS Tanto, and I like it very much. I was putting off reviewing it till I was able to use it a little more, but I'll just add a bit to my review later.
Originally, I tried to get a sheath through Cold Steel for the Tanto, they make a kydex one, and I ordered it, but turns out they changed blade sizes (Went from 6 inches hilt to tip to 6 inches sharpened) and my knife did not fit it. I am now very glad it did not. Plus, Eric's list price is cheaper than Cold Steel's.
The sheath Eric made for me is a sleak design that hugs the blade well. It is one-piece Concealex, with nothing extra to get in the way wherever you wear it. The belt loop is an ingeniously molded piece of thicker Concealex that will snap on to your belt easily, and hug the knife tightly to you, without any danger of accidentally slipping off. The loop is removable, and can be attached to accomodate up/down, left or right side wear. It can be worn inside the waistband, or outside the waistband with equal ease. It does not have a second fastener to keep the blade in, though it fits well enough I doubt it would need one. I believe Eric would have been happy to add one had I asked for it.
Overall, I am extremely happy with Eric's product. The worst part of the whole deal was waiting for the mail man to bring it (And for some reason, he decided I should wait longer than normal... delivery date was a month after the postmark) I would reccomend his services happily, and will look to him in the future should I find my self with naked blades I wish to sheath.
