Online makers mark registry updated, now accepting digital submissions

I've added another touch mark to the page, as well as updating it.

I've also, due to several requests I'm opening the registry up for digital submission.

To submit digitally, just send me a high quality scan or picture, and the information on the form

I'll update the registry every few days as submissions come in, during the hours that I'm not trying to make a living

I've not been pushing this very hard, since I've been pretty busy, but hey, now you guys shouldn't have any excuses not to submit

Alright, to take the work out of it even further, I've created an online form and upload so that you don't even have to send an email to get your mark up on the registry. just fill out the form, and upload the image at the prompt.

I need to add the phone number blank to the form, I guess I must have missed it when I was writing the script.
