Very seldom, but it occasionally happens, the pivot screw will rotate with my blade, causing the screw to loosen.

Suggestions?

Use blue loctite on the pivot.
Using loctite on the frame screws will void warranty, the pivot however is fine.

That is correct. Every folder leaves the ranch with a small amount of blue Loctite on the pivot screw. Folks will disassemble them and reassemble without reapplying.

Is it necessary to undo the pivot screw in order to change the G10 scale?

To be 100% sure that your pivot will not ever come loose again I would suggest jb weld in the pivot and then steak screw head using a large punch.

I would not recommend that as it would probably void your warranty and it's really not neccesary. Just a bit of blue Loctite.
