What pouch(es) do you attach to your kydex/plastic sheath for kit?

Ive bought several over the years, various Niteyez, Condor, no name, molle, etc.

Has anybody found an optimal pouch that can be attached to the sheath?

Anybody make their own? (Leather, cordura, kydex, etc)
