You need a lawyer. Paid or not, that's what you need.
 
I'm sorry to hear you are having health problems and difficulty contacting your ex husband. I wish I could offer you assistance, but I'm not sure what you are wanting from us? If this is a legal matter you absolutely should contact a lawyer ASAP.
