Yes, I stitch everything by hand..
I get my supplies from http://www.nordellknives.com
For the stitching part I first use a groover to make a groove in which the thread will sink. Next comes the stitchmarker to mark the distance between the holes. I have a wheel, but a fork or will do the job also. Next I punch the holes by hand with an awl from both sides (as the awl is conical, and I want a round hole). Then it's ready to sew it up using 2 needles and a pair of pliers to pull the needle through. I like to keep the holes as tight as possible to prevent dirt and moisture to enter, and to lock the waxed thread into place more firmly.
