Which portable sharpener would be best to have if you could only carry one? Spyderco sharpmaker stone medium grit?

I'm a bit confused. Are you talking about the sharpmaker with the medium stones, or do you mean just carrying one of the stones? The sharpmaker is very much portable with both sets of stones.
 
If you're looking for something that can be used in the field, take a look at the work sharp field sharpener. Awesome little all-in-one system. If you'd rather have just a single stone, look for a smallish handheld India stone. Personally, I feel that the sharp maker stones are a bit too fragile to carry individually. I've dropped one from a few feet before with disastrous results.

Agree.

Sharpmaker stones are too fragile for 'field carry' outside of the case.

Something like a Work Sharp Field Sharpener or DMT DiaFold Double-sided Coarse/Fine would be best for 'only-one' carry.

In addition to smallish India stones, you can also get 1/2" round rods in ceramic or silicon carbide for <$10.

These will all work well but honestly the market is saturated with pocket sharpeners that will all also work well. Sharpening on a small stone takes some practice and you just need to find one that works for you.
 
The Lansky Puck isn't a bad option either. Esp if you need a good working edge on a variety of tools - anything from bushcraft blades to hatches and axes. Good option for chisels too.
