Not necessarily. Where I work, I would need to shut that conveyor down, lock it out, get the box, and then start the conveyor again. If the rules are similar there, shutting down that conveyor might involve shutting down half the facility! It's not that the camera operator is necessarily a bad employee, it may be that rules prohibit him or her from just reaching in there.

I would also have caught some footage of the box going round and round before I did anything. How do you know the employee didn't then go to a supervisor and explain the problem?

Most likely something heavy and it was placed on one side and when it moved too fast and jumped up a bit, it gained momentum and it's constantly going to spin like that because there's enough inertia to keep it moving.
