Might search on the Case Medium Toothpick pattern. Here's an example, a 510094 SS (closed length is 4.25"; don't see specs on blade length yet, but it should be close):
At the upper end of your blade length limit, Queen makes a toothpick in D2, with a 4" blade. Closed length is about 5". Most 'large' toothpicks will be in this size ballpark, some slightly larger.
That's a Queen. I have that particular knife and it's very nice @ ~$65
Also, check out the GEC #12 for around $15 more.
There is a veritable plethora [Thanks, Howie.] of nice Case toothpicks. The ones I have are the tinys but I believe they make a medium, but don't have the variety of handles.
I've got one of the Queen toothpicks, and one of the GEC. The Queen is huge, as stated earlier, and is really nice in D2 and Cocobolo. The GEC is quite a bit smaller, but in 1095 steel it's a real gem. You can't go wrong with either! :thumbup:
