Suggest me a knife

I have moras, esee's, ontario's, and a hand full of others big and small but have still not found that perfect knife for me
 
There are alot of customs under $150, good ole 1095!

for a 5" blade at 3/16" thick, he may have to raise his budget to $200. But then, you are opening up a whole new world of multiple steels.

Go custom.
No problem getting a knife, that corresponds with most of your requirements. As seen below. App. the same size, laminated carbon steel.
With the right knifemaker, you will get a knife like you want it.
The trick is finding the right knifemaker.



 
Most folks here would recommend an ESEE or Becker but your post tells me no one is going to be able to recommend your "perfect knife". You just need to spend some time at a knife shop handling knives that meet your very specific criteria.
 
Definitely look at the knifemaker offerings or find one you like and go custom. You may need to push your budget a little bit but it shouldn't be much more. Spend a couple hours looking at what has been on offer in Fixed Blades For Sale, get some ideas then go for it.
 
I really like supporting custom makers and will recommend them and choosing a custom knife. Other than that there are still a lot of production knives. Check out Scrapyard, Swamp Rat and then the Esee or becker always comes to mind. I really like the Esee 5.
 
Sounds like a TOPs B.O.B. is pretty close. Just half an inch short of what you're asking.
 
the TOPS B.O.B seems really close to what you are looking for. 3/16" thick, kydex sheath, 4 5/8 blade, micarta scales, 1095 steel, modified scandi grind and just about $100
 
Try the knives of J.Martinni, Finland. Especially the leukus. They have a good website and knives with similar specs to what you cited. I know first hand they are very good knives and very reasonable in price.

The suggestion for the B.O.B. is solid too. Good knife.

Scandi grind and a 3/16" spine thickness seems counter intuitive to me. I associate scandis with thinner spined bushcraft blades in the 3/32 - 1/8" spine thickness category. You'd think a 3/16" thick blade is destined for somewhat rougher use, including batoning. I wouldn't like a scandi grind for batoning. No sir.
