P-M, that sheath looks it was made by Nicolai Brix, a European kydex-bending brother. At least, the wavy edge scalloping in-and-out around the eyelets was a feature he often used on the kydex pants I've seen made by him.
 
Concur with RokJok on the sheath. It's Brix and he does nice work.
