The original movie hawk was made by Sword & Stone.

A repro, featured on the cover of Blade magazine - Dec. 2000, was made by Joseph Szilaski.

On who could/would make one for you today I'm at a loss as I have no experience with custom makers. Non-the-less, here's a list of people I would expect to be potential qualified makers.

a while ago american tomahawk company was talking about producing one, don't know if/when it's still happening
