The handle on this knife is a smooth delrin material, which may have too little grip when wet. What would be a good way to improve the grip, sanding? or maybe a cord wrap?

you could use a thin round file to carve some shallow channels into the delrin around the circumference of the handle, spaced 1/2" apart. Or use a dremel to "jig" the delrin like bone.
 
It looks as though those scales are attached with screws. If they are removable, why not make another set in micarta?
 
Not sure exactly what worries you.. I mean, the knife has double guard after all.
 
Tennis racket wrap.

Rustoleum rubber protective Coating spray or dip.

Textured vinyl/laminate wrap or strips.

Drill small holes. Add rivets/buttons.
