I've got 4 small custom fixed blades that I carry (not at the same time) between my belt and my pants on my weak (left) side with my shirt pulled over it.
3 out of 4 of them are various levels of uncomfortable being up against my skin. The comfortable one is a Tom Krein TK-4 because it has nice smooth micarta handles. The others are skeletonized (2) and cord-wrapped (1) and eighter scratch, dig or abraid my side throughout the day. I wore the cord-wrapped one that way ONCE. The cord is impregnated with something to make it water-resistant and I cannot describe how painful it was by the end of the day.
What we need is a sheath design where a piece of well-finished (smooth/rounded) kydex extends up behind the handle to be between the handle and human body. This would also help guide the blade back into the sheath which is pretty hard to do with high-ride sheaths.
