I'm trying to describe a sheath I need made and not sure what the clock references represent in relation to where I want to carry the knife.

When someone says "3 oclock" do I look down past my large stomach and my belt buckle would be 12 oclock and then travel along my belt to the right and my right hip would be 3 oclock?

Thanks for the education

Yep. Your torso would be the clock face looking down on it. Belt buckle 12:00...The leather blue jeans brand patch is pretty much at 04:00 (YMMV) one's butt crack :o 06:00 etc...

Right handed CCW carriers are big on the IWB 04:00 position. Horizontal knife carriers are big on I'd call it 10:00 or 11:00...right there above the left pants pocket for a cross draw.
