Recommendation? Where to find rigid gift boxes for folders?

Hey guys... doing my first run of 20 folders and trying to figure out where to get the boxes which will be matte black have my logo heat stamped on the lid. I'm thinking something along the lines of what Chris Reeve or others do, with cut out foam inserts inside.

So far I have found these, but I will need to source the foam (and where to cut it) and somewhere to heat stamp the logo on top. Any suggestions? Anyone else went through this? Most places have minimums.
