The sheath looks like a Dawson knives sheath.

Looks like a Criswell (sp??).

I have a couple of his smaller blades.

Win Heger used to sell some of his knives.

Hard to tell, they all look alike, but if you search azwelke kydex, member here, you'll see he's made some that are very similar to that one.

Don't know if you bought it from him, but Dan Huang had this at one point. He may know who made it.
