Wondering if and how much for a knifemaker to make me this?

I'm not sure of the Victorinox model but it's the small knife with a blade, scissors, might include a nail filer, and a tweezer and a toothpick. But I want one of those with a Mammoth Ivory handle on both sides. How much would something like that cost me for a knifemaker to do? Thanks.
 
It sounds like either the Classic or the Ambassador.

I know SOSAK would be a decent place to find out who could do the mods (it looks like rotokid isn't taking orders now)
 
Here I'll save you the trip to SOSAK. Rotokid is ill at the moment so he is not doing many (any) mods, but might in the future. Your choices are Scibeer http://e-webtechnologies.com/knifemods/ who does good work but tends to stay more in the standard SAK look, and knifecoop http://www.knifecooperative.com/ who also does good work but tends to make SAKs look more like traditional folders. I'm not sure who has the time or what it will cost you to have your custom done. You should check out the Victorinox (and Wenger) site(s) and pick out the model you want them to modify (Classic, Classic SD, Rambler, Ambassador or Executive all from Vic or the Esquire or EVO 81 from Wenger).
 
Let's move this over to the embellishment area where its a better fit.
 
Another option is from Rotokid. He does provide blank (shaped already, but some works needed) alternative for the scales. Check out sosakonline.com
