I'm voting for Victorinox. I've had a lot of their products over the years and I've always been amazed by their consistent quality.

AFAIK all Fallkniven knives are manufactured in Japan. I like their knives but they're not really manufacturing them right?

I would add Blade-Tech to the U.S. list. they may not have a lot of products, but the ones they ahve are top notch quality.

Camillus makes some fine knives as well. I know that they have a wide range of quality.

Switzerland: Victorinox, Klotzli
U.S.: Chris Reeve, Benchmade, Pro-Tech, Leatherman, Spyderco.

I know Benchmade has some of its products made overseas, but so does Spyderco. I still include them, however, because they do manufacture a lot of products here and the quality is excellent.

That's true, but you could say the same for many of Spyderco's knives as well. I was just thinking about which companies I considered to be top notch. Where they were made didn't enter into my thoughts at all.
