If you drop a large sharp chopper on your toes, it may not make much of a difference if you're wearing shoes or not (other than steel-capped boots). Perhaps with bare feet the cut will be cleaner and it'll be easier for the surgeon to re-attach the toes! ;-)
I did once drop a very sharp Spyderco Temperance (the original fixed blade model) on my foot as I was sitting messing with it on the edge of the bed. I felt the knife bump the top of my foot, thought "thank God the point didn't hit me", then looked down to see a tiny fountain of bright red, arterial blood reaching knee-height.
