Backstitching Disaster!

After about an hour of hand stitching on a sheath I'm making, I come to the point to where I need to backstitch. I made one back stitch, pull it tight, and SNAP! My thread broke. I think my needle pierced through the thread on the backstitch and broke it. Does anyone have any tips on how to end a stitch so it secures very strongly and looks nice? I've thought about punching a rivet over the last stitch, but I'd prefer not to. This is so frustrating. Help!
 
Back stitching two or three stitches is the best way. You just have to make sure the holes you make for the last two stitches are large enough so that you won't be piercing through the thread.
How are you making your holes?

I've never had thread break like that.
