No steel insert (I'm assuming you mean on the lock bar?) Yes, the box and papers are included. Below is a link to a picture of the centering.

FYI, this is the smoothest, strongest flipper I've ever felt. It's really in incredible.

Centering is close but not perfect. Never been sharpened. Factory edge.
